What to bring to your tax appointment:


Income:
· W-2s and/or 1099s
· Interest and Dividend Statements
· Pension or Retirement Statements
· Social Security Statement
· 1099R
· 1099G
· Stock Sales Information
(also date of purchase, number of shares, and cost per share)

Expenses:
· Medical out of pocket expense
· Medical insurance you pay yourself (Not paid through payroll deduction)
· Long Term Health Insurance (need company name & policy number)
· Medications
· Doctors visits
· Glasses
· Dental
· Property Tax Statement
· Mortgage Interest Statement
· Refinance Papers if you refinanced the loan in 2004
· Charitable Giving Statements
· Non-Cash Contributions be sure to have a receipt and value of donation
· Unreimbursed Employee Expenses
· Union Dues, Uniforms, Travel, Education, Home Office

Children:
· Day Care Costs
· School Supply Receipts
· Eductional Courses, Camps, or Software
· Music Lessons and/or Instrument
· College Tuition and Books

Records Retention: · Keep Tax Returns for 7 years
· Keep House Purhcase papers until you sell the house
· Keep Records of major improvements to the house until the house is sold
· Keep Stock purchase information until the stock has been sold
